Delayed removal despite signs of infection. Continued use of a line after these warning signs, whether from inattention or a failure to connect the clinical picture to the catheter, extends the infectious burden and worsens outcomes.
Failure to reassess line necessity. As noted above, a line that stays in for convenience — because replacing IV access is inconvenient, or because no one documented the daily necessity assessment — after it is no longer clinically required represents an avoidable extension of risk with no corresponding clinical benefit.
