Run it too heavy and the patient bleeds — intracranial hemorrhage, cannulation-site hemorrhage, or diffuse mucosal bleeding that can itself become life-threatening. The standard of care requires a defined anticoagulation protocol with frequent, protocol-driven monitoring — activated clotting time or anti-Xa levels checked on a fixed schedule, with documented dose adjustments in response to trending values. Intracranial hemorrhage deserves particular attention because it is both a leading cause of ECMO mortality and one of the complications most directly tied to anticoagulation management. A credible expert opinion has to separate the expected complication profile of the underlying disease and the therapy itself from the incremental harm caused by a specific deviation — an unmonitored anticoagulation gap, a missed distal perfusion catheter, a delayed cannulation-site bleed recognition — and connect that deviation to a measurable clinical consequence: a stroke that occurred during a documented period of supratherapeutic anticoagulation, a limb amputation following unrecognized ischemia, or a death following an oxygenator failure that alarm logs show went unanswered for an extended period.
Critical Documentation for Case Evaluation
