Contrast-enhanced CT is not indicated in the first 72 hours in a patient who is responding to initial management, because necrosis is frequently not yet demarcated and unnecessary contrast exposure carries its own risk in a volume-depleted patient. The standard of care calls for CT (or MRI where contrast is contraindicated) when a patient fails to improve clinically after 48 to 72 hours, deteriorates, or when a complication such as infected necrosis is suspected later in the course. A CT obtained too early can be a red herring in litigation; the more relevant documentation failure is usually a delayed CT in a patient who was clinically worsening for days without repeat imaging to reassess for necrosis or its complications.
Recognizing and Managing Infected Pancreatic Necrosis
Clinical suspicion for infection. Infected necrosis should be suspected when a patient with known or suspected pancreatic necrosis develops new fever, rising white count, or clinical deterioration, particularly after the first two weeks of illness, or when gas is identified within a necrotic collection on CT — a finding essentially diagnostic of infection. Percutaneous fine-needle aspiration for culture is no longer routinely recommended in most cases, because clinical and imaging findings are usually sufficient and a negative aspirate does not reliably exclude infection; the more common liability pattern is failure to reassess for infection at all in a patient who is clearly failing to improve.
The step-up approach. Current guidelines strongly favor delaying invasive intervention for infected necrosis until the necrotic collection is walled off, generally around four weeks from onset, unless the patient is critically unstable and cannot be supported medically that long. When intervention is required, the accepted algorithm is a minimally invasive step-up approach — beginning with percutaneous catheter drainage or endoscopic transluminal drainage, escalating to minimally invasive necrosectomy only if drainage alone is insufficient, and reserving open surgical necrosectomy as a last resort. Published trial data comparing the step-up approach to primary open necrosectomy show materially lower rates of new-onset multi-organ failure, incisional hernia, and death with the staged, minimally invasive strategy. A patient taken directly to open necrosectomy without a documented attempt at drainage first, or a patient whose infected necrosis is recognized but who is not referred to a center capable of performing image-guided drainage, presents a clear deviation from the current treatment algorithm.
Timing errors in both directions. Liability arises both from intervening too early — performing necrosectomy on unorganized, non-walled-off necrosis in a patient who could have been stabilized medically, which is associated with higher complication rates — and from intervening too late in a genuinely unstable, septic patient for whom guideline-preferred delay is not appropriate. The standard of care requires individualized clinical judgment layered on top of the general four-week framework, and a well-supported expert opinion distinguishes between the two failure modes based on the patient's actual hemodynamic and septic status at each decision point.
