Laboratory findings provide supporting but non-confirmatory context. Normal inflammatory markers do not satisfy the diagnostic obligation when the clinical presentation and risk profile warrant MRI.

Treatment: Surgical Urgency and the Neurological Window

Once SEA is confirmed on MRI, the standard of care requires urgent surgical consultation. For patients with neurological deficits or a collection accessible to surgical drainage, emergent surgical decompression and drainage with concurrent antibiotic therapy directed at the likely organism spectrum is the standard treatment.

Medical management with antibiotics alone, without surgical decompression, is reserved for a narrow subset of patients meeting specific criteria: no neurological deficits, no surgically accessible collection, and the logistical ability to perform repeated neurological examinations at short intervals with immediate operative backup.
