Emergency providers who administer one or two doses of benzodiazepines and then wait — either for specialist consultation or out of uncertainty — allow SE to progress further along its devastating natural history. The guidelines are explicit: if first-line therapy fails, second-line medications must be administered promptly. Waiting for a neurologist to arrive before initiating fosphenytoin when a patient is still seizing 20 minutes into the episode is a deviation from the standard.

Inadequate monitoring and reassessment compounds treatment failures. Patients in SE who receive treatment require continuous pulse oximetry, cardiac monitoring, repeat neurological assessments, and clinical reassessment for seizure termination. Fragmented documentation and nursing notes that fail to capture ongoing seizure activity can obscure the clinical timeline, making expert reconstruction of events essential.

Status epilepticus cases benefit from strong causation frameworks because medical literature has clearly established the relationship between treatment delays and outcomes. Each additional minute of uncontrolled seizure activity causes progressive neuronal injury through excitotoxicity, hypoxia, and metabolic derangement.
